Nurse Mary is the only victim Dexter kills who explicitly claims to be acting out of mercy ("I was helping them"). Usually, Dexter encounters killers who are selfish, sadistic, or greedy. Nurse Mary, however, believed she was an "Angel of Mercy."

Dexter stabbed her multiple times in the chest/stomach. In Original Sin , he is shown smiling as he feels her heartbeat fade.

In fiction, "Angels of Mercy" often claim they are ending suffering. Dexter’s code usually demands he stop people who cause suffering. In this scene, the interesting feature isn't the violence, but the philosophical argument.
